Alright, it is pretty easy, so it should take no time at all. You need one envelope cut with the scallop envelope die from Stampin' Up!. You need another piece of CS that measures 4 1/4 X 7 1/2. You also need a piece that is 1 X 5 1/4 that is not pictured. The color I have here is apricot appeal.
You will need to score the CS piece at 1/2 inch on 3 sides, 2 long sides and 1 short side.
Little Tip : I score on the right side of the Scor-Pal (All That Scraps), so that I can hold it tight with my left hand.

The scoring step is first, but picture wise I had to do the other one.
OK, now you are going to put the end with the 1/2 inch score line in the scoring tool, and score it again at 2 3/4. Then you will cut it like it is in the first picture on the score lines, but leaving the little scored section.

Now it is time to alter the envie. See where my finger is pointing? That corner of the envie needs to line up at the 2 3/4 mark, and then you cut the flap on the envie off as pictured. Now fold all the flaps in, except the scallop edge.

Now, add your sticky strip to the ENVELOPE. This is important if only for the fact, that the front will look more finished. I stick the bottom together first, and then do the sides. The scallop edge gets folded down in the front. Now, you can score a line 2 inches from the top, and punch a hole with the 1 3/8 circle punch.

Now with your smallest piece, you need to score it at 1/2, 1, 4 1/4, and 4 3/4. Stamp the front, and fold as shown. Using sticky strip, adhere the one end to the back of the holder, and then keeping it straight, adhere the other side.
Here is my Diet Coke bottle filled with water again...LOL. Wouldn't this be a great thing to give to team mates before a basketball tournament? Each person would get a bottle to refill, and 5 packets to flavor the water with! Or, you could have a some stamping friends over and give them each a beverage. Course, I would recommend giving them an unused bottle...hehehe.
